چکیده

PERICARDIECTOMY for constrictive pericarditis was first successfully performed just over 50 years ago' and has subsequently become the accepted definitive method of treatment. Postoperative clinical evaluations during this time have indicated good to excellent results in the majority of patients.2 8 Despite the wide use of cardiac catheterization in the preoperative evaluation of patients with constrictive pericarditis, there have been only scattered reports of postoperative hemodynamic studies.9 20 Moreover, the data obtained and conclusions reached have varied considerably, from the demonstration of normal findings9 10 to the conclusion that restitution of completely normal cardiac function seldom occurs.1" 21 The postoperative evaluation of these patients has been complicated by disparate opinions regarding the proper incision to provide optimal surgical exposure and the extent of decortication necessary to decompress the heart adequately.2 8-14 22 27 The purpose of this report is to present the hemodynamic findings and clinical results in 14 patients following pericardiectomy for constrictive pericarditis.

Constrictive pericarditis after surgical closure of atrial septal defect in a child.

Constrictive pericarditis following an open cardiac surgery for congenital heart disease is very rarely encountered in children. We report a case of constrictive pericarditis, diagnosed at 22 months after surgical closure of an atrial septal defect, in a 14-year-old boy who presented postpericardiotomy syndrome. Cardiac catheterization revealed uniformity of elevated diastolic pressures in all ...

Constrictive pericarditis complicating cardiac transplantation

Constrictive pericarditis is a disease characterized by progressive pericardial fibrosis. If left untreated it can lead to progressive heart failure and can be severely disabling. Medical management with non-steroidal anti-inflammatory drugs in combination with colchicine is promising in the acute phase of the disease but for more chronic cases pericardiectomy offers the best chance for hemodyn...

Constrictive pericarditis in a 19-month-old child.

We report a case of chronic nonspecific constrictive pericarditis with myocardial involvement in a 19-month-old infant. The patient underwent total pericardiectomy and had irreversible hemodynamic instability. Constrictive pericarditis is rare in childhood. It may follow several processes, most frequently an infectious disease. The natural course of the disease consists of progressive myocardia...

Pericardiectomy for constrictive pericarditis with postoperative increase of tricuspid regurgitation

We report a case of tricuspid regurgitation (TR) that increased markedly after pericardiectomy for constrictive pericarditis. Preoperative mild-to-moderate TR increased to severe following surgery. The patient was asymptomatic, and gradual regression of TR was observed. Eighteen months postoperatively, a left atrial thrombus formed, and a second surgery consisting of thrombectomy and tricuspid ...

Restoration of normal intracardiac pressures after extensive pericardiectomy for constrictive pericarditis.

CHRONIC constrictive pericarditis produces a characteristic clinical picture' that results from a characteristic eirculatorv alteration, namely, restriction of ventricular filling. Pericardial constriction is accompanied by typical abnormalities of right atrial and right ventricular pressure-pulse contours,2 and although the majority of patients are improved clinically after pericardiectomy, so...
